Provide any one of the following for each child needing care:<\/p>\n<ul>\n<li>Child\u2019s birth certificate<\/li>\n<li>Proof of child\u2019s Medicaid eligibility<\/li>\n<li>S. passport<\/li>\n<li>Lawfully admitted alien document (I-94, I-94A, I-97, I-551)<\/li>\n<li>Certificate of citizenship or Naturalization<\/li>\n<\/ul>\n<p><strong>Family Size Documentation <\/strong>\u2013 Provide one of the following for each parent\/guardian and child in the household:<\/p>\n<ul>\n<li>Social Security card<\/li>\n<li>Birth certificate<\/li>\n<li>Driver\u2019s license<\/li>\n<\/ul>\n<p><strong>Household Income Documentation <\/strong>\u2013 Provide documentation of ALL earned and unearned income for ALL household members, as applicable. This may include:<\/p>\n<ul>\n<li>Paystubs \u2013 current and consecutive paystubs showing the most recent four weeks of pay. Paystubs must include the gross income and number of hours worked during each pay period.<\/li>\n<li>\u2013 If you have not been employed long enough to have four weeks of paystubs, your employer can complete a VOE.<\/li>\n<li>Child Support \u2013 documentation of the child support payments received and paid out for the most current four-week period.<\/li>\n<li>Alimony \u2013 documentation of alimony payments for the most current four-week period.<\/li>\n<li>TANF \u2013 documentation of TANF income<\/li>\n<li>Veterans Benefits \u2013 documentation of any Veterans Benefits<\/li>\n<li>Social Security benefits\/Supplemental Security Income (SSI) \u2013 documentation of benefits<\/li>\n<li>Proof of Self-Employment Income \u2013 current year\u2019s Federal tax return and a that shows the hours worked for the last four weeks.<\/li>\n<li>SNAP benefits, listing all household members<\/li>\n<li>Reemployment assistance\/Reemployment compensation benefits \u2013 documentation of benefits<\/li>\n<li>Temporary Cash Assistance \u2013 documentation of benefits<\/li>\n<li>Adoption subsidy &#8211; documentation of benefits<\/li>\n<li>Worker\u2019s compensation &#8211; documentation of benefits<\/li>\n<li>Retirement benefits \u2013 documentation of any additional retirement benefits<\/li>\n<\/ul>\n<p><strong>Separation\/Divorce Documentation, if applicable <\/strong>\u2013 Provide the following if the child\u2019s parents are separated or divorced:<\/p>\n<ul>\n<li>Copy of Separation or Divorce papers or a notarized statement<\/li>\n<\/ul>\n<p><strong>Work Exempt Documentation, if applicable<\/strong> \u2013 If a parent\/guardian is exempt from work due to disability or retirement, provide any one of the following:<\/p>\n<ul>\n<li>A <a href=\"https:\/\/elc-marion.org\/wp-content\/uploads\/2017\/06\/Verification-of-Disability-3_14_14.pdf\">verification of disability form<\/a> completed by a physician<\/li>\n<li>SSI Disability award letter from the U.S. Social Security Administration<\/li>\n<li>Retirement income benefits from the U.S. Social Security Administration<\/li>\n<\/ul>\n<p><strong>Education\/Training Program Documentation, if applicable <\/strong>\u2013 If a parent\/guardian is enrolled in high school, a GED program, technical or vocational program, associates degree, or bachelor\u2019s degree program, provide any one of the following:<\/p>\n<ul>\n<li>A copy of your current, official school schedule from an accredited educational institution<\/li>\n<li>Proof of enrollment, including the class schedule, on the letterhead of an accredited educational institution<\/li>\n<li>Completed<\/li>\n<\/ul>\n<p><strong>Zero Balance Transfer Form, if applicable<\/strong> \u2013 If a parent\/guardian has previously received Florida School Readiness child care subsidy, provide the following:<\/p>\n<ul>\n<li>Zero Balance Transfer form from previous child care provider<\/li>\n<\/ul>\n<p><strong>Child Care Authorization Referral, if applicable<\/strong> \u2013 If a parent\/guardian receives TANF, Relative Caregiver Funds, Guardianship Assistance Program (GAP), Transitional Child Care (TCC), Transitional Education (TED), Protective Supervision\/Investigation, or Foster Care, the following must be provided:<\/p>\n<ul>\n<li>Correctly completed Child Care Authorization form from the referring agency<\/li>\n<\/ul>\n<p>If a required document is not available, please contact the Coalition for additional documents that can be provided.<\/p>\n<p>Depending on your individual circumstances, additional documentation may be required.<\/p>\n<p>\u00a0<\/p>\n<p>\u00a0<\/p>\n<p>\u00a0<\/p>\n<p>\t\t\t\t\t\t\t\t\t\t\t\t\t\t\t<svg viewBox=\"0 0 320 512\" xmlns=\"http:\/\/www.w3.org\/2000\/svg\"><path d=\"M31.3 192h257.3c17.8 0 26.7 21.5 14.1 34.1L174.1 354.8c-7.8 7.8-20.5 7.8-28.3 0L17.2 226.1C4.6 213.5 13.5 192 31.3 192z\"><\/path><\/svg><br \/>\n\t\t\t\t\t\t\t\t<svg viewBox=\"0 0 320 512\" xmlns=\"http:\/\/www.w3.org\/2000\/svg\"><path d=\"M288.662 352H31.338c-17.818 0-26.741-21.543-14.142-34.142l128.662-128.662c7.81-7.81 20.474-7.81 28.284 0l128.662 128.662c12.6 12.599 3.676 34.142-14.142 34.142z\"><\/path><\/svg><br \/>\n\t\t\t\t\t\t\t\t\t\t\t\t<a tabindex=\"0\">What Happens after I Apply?<\/a><\/p>\n<p>Once you have completed and submitted your application, Coalition staff will follow up with you by email within 20 days. If you are potentially eligible, you will be placed on a wait list until funding is released.<\/p>\n<p>Once funding for your child(ren) is available, you will be contacted by email for additional information about enrolling with a <a href=\"https:\/\/www.fldoe.org\/myfloridaschools\/\" target=\"_blank\" rel=\"noopener\">School Readiness contracted child care provider<\/a>. If you do not enroll with a School Readiness provider within 30 days of funding being released, your application will be dropped. If you are on the wait list for six months, you will have to revalidate your application to keep it active. Your School Readiness provider will:<\/p>\n<ul>\n<li>Support you as your child\u2019s first teacher<\/li>\n<li>Provide you with child development information<\/li>\n<li>Use a developmentally appropriate curriculum<\/li>\n<li>Provide opportunities for parental involvement<\/li>\n<li>Help prepare your child for success in school<\/li>\n<\/ul>\n<p>The Coalition contracts with licensed child care centers, license-exempt faith-based child care centers, and family child care homes. Your child care provider may have additional fees that are not covered by School Readiness financial assistance. Providers must provide you with a list of fees charged, which may include registration fees and differentials. The sooner children receive support for developmental concerns, the better the outcomes will be because children\u2019s brains are developing most rapidly in their first five years.<\/p>\n<p>Each School Readiness child will receive a developmental screening within their first 45-days of enrollment and annually thereafter. This screening will assess your child\u2019s physical, social, emotional, and intellectual development. Develop good attendance habits early. Be sure to ask your child care provider about their attendance policy, and talk to them about any attendance challenges you may have. Your provider may be able to assist you with finding solutions.<\/p>\n<p>As a participant in the School Readiness financial assistance program, you are required to sign your child in and out of your child care program every day your child attends and at the end of each month. An authorized family member or friend may also sign your child in\/out. Signing a child in\/out when they have not attended is fraud. To change providers, request a Zero Balance Transfer form from your current child care provider, showing that you have satisfied your copayment or entered into a payment plan. You will continue to receive School Readiness services during this time.<\/p>\n<p>Intentionally deceiving, omitting, or misrepresenting information in order to receive School Readiness benefits is fraud. If you are potentially eligible, you will be placed on a wait list until funding is released.<\/p>\n<p>Once funding for your child(ren) is available, you will be contacted by email for additional information about enrolling with a <a href=\"https:\/\/www.fldoe.org\/myfloridaschools\/\" target=\"_blank\" rel=\"noopener\">School Readiness contracted child care provider<\/a>. If you do not enroll with a School Readiness provider within 30 days of funding being released, your application will be dropped. If you are on the wait list for six months, you will have to revalidate your application to keep it active. Your School Readiness provider will:<\/p>\n<ul>\n<li>Support you as your child\u2019s first teacher<\/li>\n<li>Provide you with child development information<\/li>\n<li>Use a developmentally appropriate curriculum<\/li>\n<li>Provide opportunities for parental involvement<\/li>\n<li>Help prepare your child for success in school<\/li>\n<\/ul>\n<p>The Coalition contracts with licensed child care centers, license-exempt faith-based child care centers, and family child care homes. Your child care provider may have additional fees that are not covered by School Readiness financial assistance. Providers must provide you with a list of fees charged, which may include registration fees and differentials. The sooner children receive support for developmental concerns, the better the outcomes will be because children\u2019s brains are developing most rapidly in their first five years.<\/p>\n<p>Each School Readiness child will receive a developmental screening within their first 45-days of enrollment and annually thereafter. This screening will assess your child\u2019s physical, social, emotional, and intellectual development. Develop good attendance habits early. Be sure to ask your child care provider about their attendance policy, and talk to them about any attendance challenges you may have. Your provider may be able to assist you with finding solutions.<\/p>\n<p>As a participant in the School Readiness financial assistance program, you are required to sign your child in and out of your child care program every day your child attends and at the end of each month. An authorized family member or friend may also sign your child in\/out. Signing a child in\/out when they have not attended is fraud. To change providers, request a Zero Balance Transfer form from your current child care provider, showing that you have satisfied your copayment or entered into a payment plan. Contact the Coalition for assistance with transferring your child(ren) to a new provider.<\/p>\n<p>\t\t\t\t\t\t\t\t\t\t\t\t\t\t\t<svg viewBox=\"0 0 320 512\" xmlns=\"http:\/\/www.w3.org\/2000\/svg\"><path d=\"M31.3 192h257.3c17.8 0 26.7 21.5 14.1 34.1L174.1 354.8c-7.8 7.8-20.5 7.8-28.3 0L17.2 226.1C4.6 213.5 13.5 192 31.3 192z\"><\/path><\/svg><br \/>\n\t\t\t\t\t\t\t\t<svg viewBox=\"0 0 320 512\" xmlns=\"http:\/\/www.w3.org\/2000\/svg\"><path d=\"M288.662 352H31.338c-17.818 0-26.741-21.543-14.142-34.142l128.662-128.662c7.81-7.81 20.474-7.81 28.284 0l128.662 128.662c12.6 12.599 3.676 34.142-14.142 34.142z\"><\/path><\/svg><br \/>\n\t\t\t\t\t\t\t\t\t\t\t\t<a tabindex=\"0\">Keep Your Information Current<\/a><\/p>\n<p>It is your responsibility to notify the Coalition within 14 days if any of the following has changed:<\/p>\n<ul>\n<li>Contact information \u2013 address, email address, or phone number<\/li>\n<li>Number of household members<\/li>\n<li>Work status \u2013 job loss, new job, change in hours, etc.<\/li>\n<li>School status \u2013 no longer attending, new schedule, change in hours\/credits, etc.<\/li>\n<li>Income status \u2013 an increase in your income<\/li>\n<\/ul>\n<p>If there has been a change in your purpose of care, for example a parent\/guardian is no longer working or attending school at least 20 hours per week, you will have three months to re-establish your purpose for care. You will continue to receive School Readiness services during this time.<\/p>\n<p>Intentionally deceiving, omitting, or misrepresenting information in order to receive School Readiness benefits is fraud. Forty-five days before your services expire, you will receive an email. You will then need to log into your Family Portal account to submit your redetermination application.
